Turn One AI Incident Into a Small Recovery Check
An incident review can explain what happened and still leave the next release with nothing new to catch it. A useful renewal is to turn one verified failure condition into a small check the team can revisit.
Keep the incident timeline and the recovery steps that worked. Then, where the failure can be tested safely, create a sanitized test case that checks for the same behavior before release. If it cannot be reproduced, record a manual scenario check instead—and be clear about what it does not verify. Give someone ownership of keeping the check current, and review it after the next relevant change.
That’s a bounded way to recover useful learning without rebuilding the whole incident process. The log stays as history; one part of the response becomes a safeguard.
